How to Check Your Premium Payment Contributions

Discussed herein are methods with which to check your premium contribution payments, whether you are paying as an individual or are being remitted/paid by your employer. Individuals can request for a copy of your premium contribution history which will reflect your payments as an individually paying member or the payments remitted by your employers as posted in the PhilHealth database.
  • Phone Inquiry
    • You may contact the PhilHealth Call Center at (02) 441-7442;
    • You may call the Collection Section of the nearest PhilHealth office.
  • Online Inquiry
    • PhilHealth currently does not provide an online facility for you to view your premium contributions online. We will update this as soon as the service is available.
You may request for a copy of your premium contribution history to be emailed to you in any of the above methods. You would need to inform them of your PIN, full name, birth date and your current employer (if any).

  5. hi, nkakuha na po ako ng MDR pero nung binigay po sa akin ng HR namin, nka-indicate pa po don ung previous employer ko na 2008 p po ako wla s company na un, and I am currently working sa ibang company for more than 2 years na po. I'm about to give birth this November, and I am worried that I may not avail the benefit that I should get. May deductions din naman po ako pra s Philheath, I just want to know kung ano po dapat ko gawin? thanks

  6. Kelangan mo na i-update ang iyong record sa PhilHealth through your current employer. Fill-out a PhilHealth Member Registration Form (PMRF) at ibigay sa iyong employer para maifacilitate nya kasama ng ER2 na form. Ibibigay nya ung document sa PhilHealth para mabago.

    Maaari ka ring mag request ng Certificate of Premium Payment o kopya ng iyong contributions record para ebidensya na nahuhulugan ka ng kompanya kung hingin sa iyo ng ospital.

  7. Good day po! I've been working po since February 2006. Unfortunately I have to quit my job last April 2011. Currently I still don't have a job. Because of this po, I was not able to continue my contributions w/ PhilHealth since then. I just want to know if I can still make payment of the months that I was not able to contribute (May to October 2011) & if I still can, what should I do? Please help me.

  8. Accdg to PhilHealth rules, a one month grace period immediately following the missed quarter is provided to settle the contributions. In your case, payment for the 3rd quarter may still be accepted along with the Oct-Dec (4th Qtr) premiums.

    You would have to check directly with PhilHealth if they will still allow retroactive payment for the May-June premiums. Inform them that you missed payment due to the change in employment status but is willing to pay for the missed quarters. Contact the call center or go to the nearest PhilHealth office to directly pay the premiums.

    I just want to ask if we could avail discount for prescribed medicine in any drugstore as a PhilHealth member. If yes, how?

    Also, I want to consult my father's case. He is working right now. He was advice to undergo a six month medication for Inactive Tuberculosis. Fortunately, he was not asked by his employer to file a prolong Sick Leave. What benefits can my father avail from PhilHealth?

  12. If we are to follow the guidelines on acceptance of retroactive payments, your payment for the 3rd quarter will be applied to the 4th quarter as the grace period has expired. However, you may still directly coordinate or inquire with PhilHealth if they can accommodate your payment for the 3rd quarter.

    I left my company last May my this year. I failed to secure my PIN from them. I want to continue paying my contribution on my own since I will be needing it soon to have my baby. I know I am missing payments because of this and I want to make it up. I want to know what is the easiest way to get my PIN. Will I have to check with my previous company or can I go to the nearest Philhealth office in my area?

  15. Request for a certification from your employment that you are no longer an employee. Accomplish a PhilHealth Membership Registration Form (PMRF) to effect change in category from employed to Individually Paying - Non Professional. This will update your membership profile if you have an assigned PIN or if you are not yet registered, you will be provided your newly generated PIN. Use the issued PIN in paying for your contributions. To avail you would need 9 months contribution within a 12 month period prior availment.

    I would like to know what would be the best action I to do for this scenario:

    My father has been an active member of Philhealth for quite some time. Last year, he stopped working and so his contribution with Philhealth. He is now 61 years old. I would like to know if he can still continue paying for his contribution with self-employed as his status or can I just declare him as my dependent since he is already a senior citizen. Recently, I applied a new Philhealthy card for him when I applied mine.

  18. Q1: will still need to verify with PhilHealth. If the check issued is not cross-checked then it may be endorsed.

    Q2: if you are married and will likely declare your spouse or child/ren as dependents, then it would be better for your father to have his own membership so that the allowable 45days shared between dependents will be maximized.

  19. good day po happy new year. nag work po ako for almost 4 years nag co-contribute po ako sa philhealth. Pero its been 10 years na po ako walang trabaho, pina activate ko po yung contribution ko last month. Di ko po na gamit yung contribution ko for almost 4 years na nag trabaho ako. Pag ganito po ba yung situation maari ko pa rin po ba makuha o magamit yung na contribute ko maski 10 years ago na ang nakakaraan? ma o-operahan po kase ako by March...

  20. Happy new yr din Elizq. To answer your question, sadly hindi mo magagamit ang dati mong kontribusyon o mawiwithdraw ito para sa March. Ngunit ang dati mong kontribusyon ay gagamitin ng Philhealth na basehan kung ikaw ay maging lifetime member. Maaari mo pang bayaran ang 4th quarter at 1st qtr 2012 bka sakaling maging eligible ka magavail ng benepisyo. May mga surgical benefits na kailangan ng 3months na hulog within a 6month period. Panatiliin mong updated ang iyong kontribusyon para tuloy tuloy ang benepisyo.

  23. Q1: For OFWs, benefits can only be availed within the validity period of your payment. Hence if your validity ends on april, then you have to pay your premiums on or before the expiry date to avail the maternity benefits.

  24. Hi, good day po. I’m 20yrs old, 7mos pregnant now, will give birth on MARCH 2012. This is my first baby. I was a member of Philhealth since May-Oct 2010. Pero hindi na ako nakakapaghulog now kasi yung previous job ko hindi nagkakaltas ng Philhealth and i'm unemployed now.
    Ask ko lang po kung pwede rin ba ako makakuha ng maternity benefits ng Philhealth?

  25. For now, you are not entitled to benefits as Philhealth is requiring that you have at least 9 months cobtribution within a 12 month period prior your delivery date. However, your previous contributions will still be used to qualify you as a lifetime member when you turn 60yrs old.

    Continue paying your premiums to receive the benefits the next time you might need it.

  27. PhilHealth only allow members retroactive payments where members can present proof of nine (9) consecutive monthly payments prior the missed quarter. Members are also required to settle the payment within a month immediately following the missed quarter.

    Since one year ka nang hindi nagbabayad ng PhilHealth, you are no longer eligible for retroactive payment accdg. to their ruling. However, you may still check or confirm directly with PhilHealth through their call center.

  30. hi everybody. I need help on this matter. I was employed from December 2008 upto December 2010 as an OFW. Then i got a job in here, sa Pilipinas from January to July 2011 and my company was able to pay my contributions. Nagresign ako last month ng August matapos kong malaman na I am pregnant. Since August hindi na ako nakapaghulog. Pwede ba mabilang yung contributions ko from January to July kahit maghuhulog ako ulit this January 2012 as voluntary na? Pwede ko pa rin ba bayaran ang mga na miss kong months (August to December). Gusto ko po kasi maka avail ng maternity benefits. Manganganak kasi ako ng April 2012. Sana matulungan nyo ako.

  32. Q1:edmundo, fill up ka ng Philhealth Membership Form (PMRF) to update your profile as individually paying member and submit to any Philhealth office. You can pay the premiums in any accredited collecting agents, list ais available in the blog.

    Q2: you may contact the call center to inquire if you are now registered with Philhealth. Answer the questions that they will be asking for them to search your record.

    Q3: as to availment dapat ay may 9months kang hulog sa loob ng 12months bago and pag gamit. So kung april ka gagamit dpat from apr 2011-mar 2012 may hulog kang 9 months. Ung mga nahulog mo nman nung dati pa ay gagamitin to qualify you as a lifetime member. As for retroactive payments, nagaalow ang Philhealth provided may sufficient regularity of payment. Pero ikaw ay binibigyan lamang ng isang buwan immediately following the missed quarter para isettle ang payment.

    Q4:kung starting jan 2011 hanggang sa buwan ng duemo ay walang lagtaw na bayad, pwede mo magamit ang Philhealth benefits dahil may 9months kang hulog within 12months bago ka magavail.

  34. good day! before, ako ang humuhulog sa philhealth ng asawa ko, tapos nagwork xa as OFW then ang agency na ang mahulog. he started working nung April 2011. and when i went to the nearest philhealth office.. wala pa din hulog ung philhealth ng asawa ko.. palagi ko pina follow up ung agency, hindi pa raw posted.. what can i do about this? pwede ko ba ireklamo or kasuhan ung agency na yun (with what case)? salamat po

    Q2: Nagde-deduct ba ang agency ninyo ng PhilHealth benefit? As far as we know, for OFWs, members kasi mismo ang nagbabayad. Pwede mo ipagbigay alam or inquire sa PhilHealth kung may natatanggap silang report ng remittance nung agency and kung kasama ang asawa mo dun.   38. Previous contributions will be used to qualify a member into the lifetime program. Yes you can continue paying your premium contributions on a quarterly, semi-annual or annual basis. Pay for 2yrs for only 2,400php or 100per month within this quarter. Premiums will increase from 200per month starting July 2012. You cannot pay for your missed months however.

    1. Previous contributions are still counted for the qualification of being a lifetime member so it would be better to continue paying your contributions. To reactivate, you need to update your profile and start paying your premiums.

    1. hinggi ka ng authorization letter, id at photocopy of contract from your brother. Photocopy mo ung ID ng brother mo at ung ID mo pati na rin ung resibo na dapat i-update. Prepare a request letter to request a copy of your brother's member data record (MDR). Punta ka and submit mo sa kahit anong PhilHealth office. Siguraduhin na ang resibo na ito 11482830 ay naka-reflect sa mdr na binigay sa iyo ng PhilHealth.

  44. Hi gudam, ask ko lang po kung ano gagawin ko kpg ung previous employer ko hindi ako inapply sa philhealth pero may deduction ako para sa philhealth contribution ko. Sabi kc ng former employer ko before sila na ang bahala magforward nung mga paper or forms from philhealth na filled up na. Then after ko magresign nagpunta ko sa nearest branch nyo to get my PIN pero ang sabi wala daw ako record even sa SSS ko hindi daw nila hinulog. Kaya nagavail ako ng bago. Ngayon po ang ginawa ko nagavail ako sa province pero wala ako work that was last December 2011. I am working now but i did not passed yet my Philhealth number pero may deduction na ko, ang question ko nmn po about dito, pwede na po ba sila magremit ng contribution na wala pa akong Philheath number?

    1. Kung marami kayong anak, mas makabubuti na kayong dalawa ang magpamiyembro para ma maximize ang philhealth benefits. Sa gayun ay pwede nyo rin i declare na dependents ang bawat magulang ninyo na may edad 60 pataas.

    1. Hindi po tumatanggap ng retroactive payment ang philhealth maliban lamang sa mga nagbago ng kategorya from employed to individually paying member. Ang inyong contributions ay magiging basehan ng inyong qualification under the lifetime program.

      Makabubuti na ituloy ang paghuhulog upang hindi maapektuhan ang benepisyo kung ito ay kailanganin na.

    1. Kailangan na updated ang inyong membership record sa philhealth dahil ito ay gagamitin kung kayo ay mag avail ng benepisyo. Bago pa man mangailangan, mas makabubuti na ito ay siguruhin na updated para hindi magka problema.

      Kung kayo ay lumipat na ng trabaho, ang usual na nagpapa update ay ang employer.

    1. Depende po yan sa validity ng coverage nya. Ofws have a valid one yr coverage starting from date of payment. So before ma expire ang coverage dapat magbayad ng premiums upang hindi magkaproblema sa availment.

Self employed sya kaya hndi sya gaya ng employed na automatic nadededuct ung sa philhealth. napabayaan nia ung philhealth nia at first quarter lang ng 2011 ang nabayaran nia. ngayon pong may e naospital sya. akala namin pwede pa bayaran ung previous quarters para makaclaim pa kami ng benefits. un pala hndi na pwede bayaran un so ang gnawa nalang namin e bnyaran na namin hanggang dec 2012. wala parin po ba kaming makakuhang benefits this month? di po ba pwede icredit un? salamat po.

    1. Para makapag avail ang voluntary member, kailangan usually na may 9 months na hulog within a 12 month period bago gamitin or 3months na hulog within a 6 month period para sa ibang benepisyo.

    1. Yes pwede mo ipagpatuloy ang hulog mo ngunit dahil sa gap possible maapektuhan ang pag avail mo. Pwedeng iba ang magbayad ng iyong premiums ngunit siguraduhin na tama ang detalye na ibibigay lalo na ang pangalan at PhilHealth number

NOw, my grandma is in the hospital, and she's on of the dependents of my uncle. the question here is the birth certificate of my grandma is not found at the NSO. is it ok to pass a birth certificate from the local registry of my grandma and a scand birth certificate of my uncle together with the filled up PRMF of my uncle in your office here in cdo? plz reply. we need to comply everything before one week ends.

    1. Go to any PhilHealth office and request a copy of the Member Data Record (MDR) of your uncle. Ask your uncle to provide you an authorization letter as this will be required and if possible his valid ID. PhilHealth will accept birth certificate from the local registry attach it to the PMRF to update his record.

    1. Sadly hindi mo magagamit dahil sa lapsed na payment. Kailangan ng 3 months na hulog within a 6 month period bago ang confinement and since wLa kang hulog ng 1st and 2nd quarter, possible na di mo magamit ang benefits. Siguraduhin ang tuloy tuloy na bayad upang huwag magka problema sa availment

  68. Ang requirement for maternity benefits ng employed ay dapat may 3 months na hulog with a 12 month period bago ang availment. Since aug pa ang due mo and jul, jun, may at apr ang hulog dapat eligible ka for availment. Ensure mo lang na accredited ang hospital at doctor.

    2. Since you are currently employed, you would have no problem in availing for the criteria of sufficiency of contributions as PhilHealth is requiring 3 months contribution within a 6 month period (only for employed members) prior due date.

      You may inquire with your company if they have included you in the remittance reports being provided to PhilHealth. Possible rin naman na may backlog ang posting ng contribution sa PhilHealth due to the volume ng transactions. Hence, kung ma-confirm mo na kasama ka nga sa report na binibigay ng employer, then you can just follow-up sa PhilHealth ung posting ng contributions ninyo.

    2. employers are mandated by law to deduct from monthly salary. Pwede mo i-request na mai-offset or mai-apply sa ibang prospective months ung nadoble mong contributions.

      Pwede mo inquire sa call center or email mo ang info@philhealth.gov.ph para ma-ensure na hinulog at nireport ka ng iyong employer.

  76. i gave birth in June 2012 and when i checked out of the hospital, they said i couldn't avail of the discount because the last payment on philhealths system was in 2011 pa. But i have been deducted monthly by my employer.

    i followed it up with my employer and they said they are co-ordinating with philhealth.

    the 45 (or is it 60?) days claim deadline is fast approaching and there doesn't appear to be any record on their system yet. any advice?

    1. Since you were not deducted your PhilHealth benefit, you may try to directly file the reimbursement. Prepare and submit to PhilHealth all required documents for claiming the benefits. You need to file 2 sets of claim, one for the maternity package and another for the new born care package. File it before 30 days to be sure but filing period is usually 60days for local confinement and 180 days for confinement abroad.

    1. Ang payment mo as ofw ay valid for one year. Maaari kang mag request ng MDR para makita mo kung hanggang kelan ang validity mo pero usually ito ay one yr from the date of payment. Kung may kakayahan ka upang magbayad, maaari kang magbayad ng 1200 for next year para maka siguro na wala kang magiging problema as far as contributions is concerned. Ang premium na 1200 ay hanggang end of September lamang. Kung ikaw ay nagbayad ng 900php last february, kailangan mo magbayad pa ng 300 para sa taon na ito. Dapat ay 1200 na ang contribution batay sa PhilHealth implemented last january this year.

Wala po ako work from May 2011 to November 2011. December 2011 i applied and was hired in another company pero hanggang 1 month lang ako, i had to stop work because of some personal matters.     1. Hindi ka na makakapag retroactive payment. However, pwede mo ituloy ang hulog mo starting 3rd quarter. Mas makabubuti na magbayad ka na hanggang 4the quarter bago mag Oct dahil tataas ang premium from 300/qtr to 600/qtr. Kung nanaisin pwede ka rin magbayad ng advance for Jan to Dec 2013 para 1200/yr at hindi 2400/yr ang babayaran mo. Maaari mo naman irequest for adjustment kung ikaw ay magkaroon na ng trabaho.

Hello po, ask ko lang, ito yung case ko..dati po ay employed ako sa isang call center company tapos na.terminate po ako..2 months lang ako naka.pag.work sa kanila pero may philhealth contribution na ako..kinakaltas nila sa sweldo ko.. Then I wasn't able to get my philhealth number. So gusto kong ipag.patuloy ang aking philhealth as self employed pero the problem is pag.inquire ko sa philhealth office wala daw akong philhealth number, baka daw hindi naka.pag.submit ng ER2 ang aking employer nuon..So ang tanong ko, is it okay if mag.apply nalang ako ulit sa Philhealth, I mean another registration?..and yung contributions ko nuon ma.add ba yun sa new Philhealth account ko? Kung makaka kuha ka ng certificate of employment or separation sa employer mo detailing kung kelan ka na-hire at kelan ka na separate. PhilHealth may be able to map your contributions sa bago mong number.

    1. as long as ininform mo ang employer mo ng iyong PIN o PhilHealth number at yun ang ginagamit niya sa pagreport ng contribution mo, hindi maliligaw ang contribution mo.   111. gud am..im Joan DUcha..i worked for dec 2005 til january 2008 sa isang electronics company..sad to say npasama aq nun sa recession that time..ask q lng qng ung philhealth conribution q for 3 yrs is active pa b?..wala akong job from then till now.but im willing to pay..pde b p me mgpamember?..or still member pa aq ng philhealth..thanks..

    2. The months that I missed, can I doretroactivepayment?
    3. If I get married, can my parents still be my dependents? Also my children and spouse?

  115. Good day po...ngwork po me since 2011 up to 2013...na end of contract po me nung feb. 2013...3 months n po me hindi nakakapag hulog..hindi po b mawawala ung contribution ko?balak ko po sanang mg individual payment for the second quarter?

  117. Good day! Ask ko lng po ano pong pwde kong gawin sa philhealth ko na nhuhulugan ko nung dalaga pa ako at ngayon nkadependents nlang ako sa. Philhealth ng husband ko. Gusto ko pong maging useful nman umg mga contributions ko sa philhealth ko. Please help me what should I do?

    Nagstart po akong mag work noong january 10, 2013 and end at april 15 2013 kasi po reliever lang po under sa agency. Noong magbabayad po ako para icontinue yung contibutions ko for month ng april, may and june and sabi po kailangan ko daw ng certificate of contributions. Anu yun? san po iyon nakukuha? Bat noong nagresign ako sa agency ko ang sabi lang nila just show ko lang yung payslip ko puwede na daw akong magtracsact.. Ano po ang magandang gawin? especially unemployed po ako hanggang ngayon. Thank you..

    1. Kailangan mong ipa-update ang iyong membership type sa Individually Paying Member kung ikaw mismo ang magtutuloy ng iyong contributions. Refer to this post for updating your record: http://philhealth101.blogspot.com/2011/04/how-to-update-your-membership-record.html

      Pagkatapos mong iupdate ang iyong record, maaari ka magbayad ng iyong premium contribution sa alinmang accredited collecting agent (usually may PhilHealth Accredited" na sign. Ang pagbabayad ay quarterly, semi-annual or annual payment para sa Individually Paying Member.

  120. tanong ko lang po kasi itong 2nd quarter last june 28 2013 po ay di nahulugan ng tatay ko yung contribution nya as individual paying member pede po ba ito mahulugan this month para maupdate po ung contribution nya?

    1. sa ngayon ay walang retroactive payments ang PhilHealth na ipinapatupad. Maaari nya ituloy ang hulog para sa 3rd Qtr at siguraduhin na dire-diretso ang hulog upang wag maabala kung sakaling kailanganin gamitin ang PhilHealth.

  121. pumapasok po ba ang philhealth contribution ko kahit wala pa po ako philhealth number? nakapagregister napo kasi ako by means of philhealth form sa company namin... I still don't have the philhealth number i asked them bakit wala pa po ako number.. sabe sakin nung HR nmin napasok naman daw po yun contribution q ang gamit nla is SSS number q wala daw po kasi until now binibigay na philhealth number sa knla para sa name ko... tama po ba ito? salamat for the reply

    1. yes, nare-record ang iyong contribution ngunit para makatiyak na lahat ng iyong contributions ay mairecord sa permanent account mo, kailangan mo ng PhilHealth number. To facilitate, pwede magregister online ang iyong employer para sa records ninyo or i-follow-up sa opisina ng PhilHealth at kaagad na gamitin ang iyong PhilHealth number.

    2. Good day! Gusto ko lang itanong, if magagamit ko na ba yung philhealth ko. Kasi ngayong march lang ako nagpagawa ng philhealth voluntary, then binayaran ko ng 1,800 for the whole year which means kahit na march ako nagpagawa i still paid the last two months January and february. I'm pregnant po kasi and i'm giving birthd this coming september.. Ang sabi 9mos. Na dapat nahulugan ang philhealth, so magagamit ko na po ba yung philhealth ko? Kahit ngayong march ko lang sya pinagawa pero i paid for the whole year na. And if ever na hindi pwede, sakop padin ba ko as beneficiary ng mother ko? I'm 19 and turning 20 sa sept.26 pero mga 1st week of september ako manganganak. Ang sabi kasi 20y.o and above yung hindi na sakop ng beneficiary, so ibig sabihin pasok padin ako as my mom's beneficiary?

  123. Good Day, Pregnant po ako now, ang duye ko po sept2013, dati po akong employed at nahulugan po ng employer ko ang premium ko from feb2011-April2013, d ko po nabayaran ung May at June ko, pero ung 3rd quarter (july-Sept) nabayaran ko na po, can i still avail the maternity benefits?

    Mag inquire lang po sana ako kasi po hindi na po kas nagpo-provide ng certificate of contribution yung last company na pinasukan pag di ka na daw nila empleyado. Pumunta po kas ak a Philhealth office at kailangan daw po nun para ma-continue ko po yung payment ko. May paraan pa po ba para macontinue ko yung payment ko without Certifcate of contribution dun sa last company ko?

  127. good day po..self imployed po ako mgkanu na po ang contribution strting 2013 kc sa byad center po ako plgi ngbbyad ang hinihinge skin 150 a month..may tanong pa po ako ng strt po ako ng self imployed 2004 my na miss po akong d nahulugan 2nd and 3th quarter..pero nghulog po ako ng 4th qrter 2004..tapos the follwing yrs nsundan nman po d ako nkpagbyad ng basta po sa isang taon d q nbbubuo byran pero my mga taon po buo ang byad q..pang 10 yrs q na po ito 2013..self imployed..pano po mangyyri sa mga na missed q d binayaran na quarters???? pls help me po!!!

    Just to clarify something, can I avail my philhealth with this kind of scenario? " I was employed last August 17, 2009 as a supervisor and unfortunately due to some personal circumstances I give up my position last May 27, 2013 with no lapses to my philhealth contributions. Along the way, I was failed to give my contribution last June 2013 (1 month only). And continue again this July 2013 up to this date (October 24, 2013) due to I employed to another company. Got a 3 child and they are all my beneficiaries. My concern is, my second child was been confined at the hospital accredited by philhealth, and Im very much worry na baka di tatanggapin ang philhealth ko due to abridge contribution last June 2013. Can I avail my contribution with this kind of scenario? Need your advise..Thanks..

Im philhealth member but got unemployed since June 2012 dahil bumalik ako sa pagaaral so bale hindi ko nahulugan yung a year and a half contribution as voluntary as well as i wasnt able to change my status to employed to unemployed kasi biglaan ang lipad ko palabas ng pinas nong nitong May and im 7 months pregnant. andito ako now sa japan and babalik ng pinas on january 3rd para manganak. i got hospitalized here last week for 10 days. is there a change na makapagreimburseme ako sa philhealth on the expenses ko? can i pay philhealth on the months na walang hulog so i can avail the benefit and use philhealth on March sa panganganak ko?
