Members: Declaring PhilHealth Dependents - Qualifications for Valid Dependents

This blog post is for those:
  • Members who are wondering who to declare as PhilHealth dependents in the PhilHealth Membership Registration Form (PMRF), and;
  • Those whose Member Data Record (MDR) did not reflect the dependents previously declared.
Who to Declare as a Dependent:
  1. Biological Mother;
  2. Biological Father;
  3. Step Mother (if biological mother already deceased);
  4. Step Father (if biological father already deceased);
  5. Adopted Mother (with adoption papers or Court Decree/Resolution of Adoption);
  6. Adopted Father (with adoption papers or Court Decree/Resolution of Adoption);
  7. Biological Son/Daughter;
  8. Step Son/Daughter;
  9. Adopted Son/Daughter (with adoption papers or Court Decree/Resolution of Adoption);
  10. Legal spouse (non-member or membership is inactive).
Qualifications of a valid dependent:

1. For Parents (birth certificate)
  • Should be 60 years old and above;
  • Non-member or inactive member;
  • Unemployed.

2. For Spouse (with marriage certificate/contract)
  • Non-member or inactive member;
  • Unemployed;
  • A Muslim member can declare more than one wife (with attached Affidavit of Marriage issued by OMA).
3. For Children
  • Should be below 21 years old;
  • Unmarried;
  • Unemployed;
  • 21 years old and above with congenital disability either physical or mental;
Additional Guidelines:
  • A dependent is qualified if they are not registered as PhilHealth members or if their membership is no longer active;
  • Generally, PhilHealth requires the submission of supporting documents to establish the relationship of the declared dependents to the member. Dependents without supporting documents will not be visible in the Member Data Record (MDR);
  • A dependent cannot be declared more than once, as in the case of both parents declaring the same child as their dependent, or siblings both declaring their parent;
  • There is no limit as to the number of dependents that you can declare. However, you should take note of the 45 days allowable limit shared by your dependents.

  1. pareho po kaming magasawa na member ng philhealth pero ako po ay hindi na nakapag hulog simula nung umalis ako sa work 2006. pag pasok po ng taong 2007 ndi na ako nkapghulog pwede po bang isama ako ng husband ko bilang dependent niya he has our son under his membership. ako matagal na hindi naghulog even nung nanganak ako (2007) walang philhealth kasi hindi namin inasikaso pero ngayon regular ng ngcocontirbute ang husband ko. PWEDE NA PO NIYA AKONG ISALI NA DEPENDENT NIYA? THANK YOU.

  4. Pwede gamitin ang baptismal certificate instead of the birth certificate. Ang buong listahan ng mga pwedeng i-attach ay matatagpuan sa likod ng PhilHealth Membership Registration Form (PMRF).

  6. Question, nagkasakit kasi mama ko pero wala pa syang 60 years old, pero papa ko 60 na sya at pwede ko syang ienroll sa philhealth, pwede ba nyang ienroll as dependent ang mama ko? at pwede pa ba namin ifile yung mga nagastos namin sa ospital? Pa help naman po. Salamat :)

    1. Pwede mo ienroll ang mother o father mo as a philhealth member and the declare the other as a dependent. However, di mo pa pwede oreimburse ang nagastos ninyo. Kailangan may 9months contributions ang miyembro bago maka pagavail ng ibang services o packages.

I am an inactive Philhealth member for almost 4 years na.Gusto ko pong madeclare as dependent sa asawa ko.kelangan ko pa po bang magsend ng letter of deactivation para doon sa membership ko?saan ko po dapat isubmit yung letter requesting for deactivation?meron bang standard form para sa deactivation letter?pls help.tnx

    1. Yes, kailangan magsubmit ng miyembro ng rrquest letter at i-attach ito sa filled up pmrf ng asawa mo na naka declare kang dependent. Isubmit sa kahit anong branch ng philhealth. Ilagay ang rason kung bkit nais ideactivate ang account

  8. tnx for the reply po.another question po ito.ang asawa ko po ay public school teacher.last month lang ay nag update kami ng pmrf nya at doon na rin sa office ng school nila namin ipinasa.at yun nga ang naging resulta, I was not qualified para maging dependent yung anak lang namin.ok lang ba na doon ko ipasa din yung request letter for deactivation with my husband's pmrf?or dun na ako mismo magpasa sa Philhealth branch office.7 months pregnant po kc ako kaya I asked kung saan ko dapat ipasa...

    1. Mas makabubuti siguro na ideretso mo na sa Philhealth para kung okay na ay maka kuha ka na agad ng bagong MDR. Kung dati kang employed kailangan ay hindi ka na nagtra-trabaho at magprepare ka ng certificate of separation na galing sa iyong dating pinapasukan

  9. hi po, same problem here ako naman po is 7 months ng inactive at hinihingan din ako ng deactivation letter ng philhealth para maging dependent ako ng asawa ko and next month due ko na po. meron po ba kayong sample ng deactivation letter? hindi ko po kasi alam kung papano umpisahan. thanks po.

    1. Sabihin mo lang sa sulat, i woukd like to request for the deactivation of my membership due to kung anong rason for the deactivation starting when. At ilagay mo na rin ung pmrf for updating ng spouse mo na dinedeclare ka as dependent, with your marriage contract and birth certificate.

  12. matagal na po ako member ng philhealth, after ko po ba ma pa add as dependent ung mother ko pwede na po ba nya magamit ung benefits.. let say na pa add ko this week, then i pa schedule ko sya for operation next week pwede na po ba agad magamit thanks.

    1. Q1: Pwede kayong maging valid dependent kung dineklara kayo ng asawa ninyo. kailangan nya i-update ang profile nya by submitting an updated PMRF.

  14. good day po... ask ko lng po, ung asawa ko is a philhealth member, umuwi po sya nung 2007 sa pilipinas at nag bayad po ng advance hanggang 2008, but from that po di na po nya na update payments nya, valid pa po ba ung membership nya? and if ever po na valid how can we update the payments po, thank you... hoping for a sooner reply

    1. kung ang asawa mo ay nasa ibang bansa pa, pwede kang huminggi ng authorization letter para makapag transact ka on his behalf at maipa-update ang kanyang membership record sa PhilHealth by submitting an updated PMRF na naka-attach ang photocopy ng kanyang contract sa abroad. Maaari mong bayaran o ipagpatuloy ang hulog niya sa PhilHealth. Para malaman kung saan pwede magbayad, kindly refer to this post:

  15. Hello po. Gusto ko po sanang itanung kung paano po magkakaroon ng Philhealth ang kapatid ko. Member po ako ng Philhealth, d ko po ba pwedeng ideclare na dependents ko ang kaptid ko? Kasi both parents po namin ay wala na. Yung kapatid ay minor plang,9 years old. Wala po bang exemptions tulad s ganitong case? d po ba xa pwedeng ideclare n dependents ko since aq naman po ang tumatayong guardian/parents nya? Kung hnd po pwede paano ko po xa mabbgyan ng Philhealth? thank you po.

    1. Ang kapatid ay hindi po kasama sa valid dependents. Pwede po sya magpamember as long as may guardian. Meaning magre register ang iyong kapatid bilang individually paying member at magbabayad ng kaukulang premiums. Pwede mo sya ipa-register this month at magumpisa magbayad bago mag Oct para sa 3rd qtr. Kung gusto mo makatipid maaari mong bayaran before Oct 1 ang pang 4th Qtr at annual payment next yr para 100 per month pa rin ang babayaran mo. Pagdating ng Oct ang premium ay tataas at magiging 200 per month na.

    1. Sa ngayon po ay wala pang online viewing. Baka maaari kayong tulungan ng call center, tumawag kayo at inquire kung naka declare na ang inyong anak or pumunta sa pinaka malapit na opisina at magrequest ng mdr. Magdala na rin kayo ng birth certificate para kung hindi pa naka register ang anak ninyo ay maaari nyo na sya agad idagdag

  17. hi po itatanong ko lang po isa po akung single at ang parents ko po wala pa sa 60 at yung mga kapatid ko po 21yrs.old above na po sil it means di na sila kasama sa dependent ko...ehh nag aapply po ako sa abroad at makukuha ko na po yung oec ko baka by nextweek pano ko po kaya gagamitin yung philhealth eh wala naman ako dito sa pilipinas at may insurance naman sa pupuntahan ko na bansa 2yrs. contratc po ako dun..pwede ko po ba ilagay sa beneficiary ko yung mga pamangkin ko..tnx!

  18. Si Mommy po ay kaka-60 years old pa lang nung June 2012. Effective January 2013, siya ay retired na pero bayad pa rin nung kumpanya yung last quarter niya ng 2012. Puwede po ba i-declare ko na siya na as my dependent? Ano po ang kelangan na documents para magawa ito?

    1. maaari mong i-check kung qualified sya as lifetime member. kung qualified sya ay mas makabubuting as lifetime na lang sya at hindi dependent mo dahil sya ay makaka-avail ng PhilHealth benefits kahit hindi na sya magbayad ng PhilHealth. Ito ay upang ma-maximize ang kanyang inihulog na contribution. Eligible sya as lifetime member kung sya ay nakapag hulog na ng minimum of 120 months contribution.

    1. Try to prepare a request letter for the removal of the particular dependent. Ilagay ang full name and birth date of the dependent na gusto mo tanggalin.

  23. hello po, nag-apply ako as Philhealth member nung 1998 as self-employed but wasn't able to pay the monthly premium, now I want to activate my account, puwede pa po ba at papano, need ko pa ba magpunta sa main/branch office to reactivate or can I just continue paying the monthly contribution in the receiving banks? Any advice will be much appreciated. Many thanks!

  26. Hi! Tanong ko lang po kung pwede ko ipa-register yung mother and father ko sa philhealth? Both of my parents are unemployed and 50 years old na.. What are the requirements for the enrollment of their philhealth? kailangan po ba kasama ko pa sila pag pumunta sa philhealth office? May sakit na kasi yung mother ko at hindi na sya pwede masyado magpagod.. Okay lang ba na ako na lang ang magpunta at mag submit ng PMRF nila? Thanks in advance sa reply! God bless..

Ask ko lang nanganak kasi ako nung september then i used my philhealth they only deducted me P5000.00 tapos nagbayad kami ng cash with the amount of P7000.00 .. then recently nakita ko sa rates ng maternity is 8000.00 pwede ko pa bang ireinburst yung 3k? since may binayaran parin kaming 7k.. thank you!

  30. Mam/ sir, ask ko lang po kung pwede bayaran yung last year n hulog 2013 at 2014 sa philhealth? Paano po process? Nasa taiwan po kasi ako. Wala ako mautusan n mag aasikaso maghulog sa philhealth ko.
